APPLICANT REQUESTS THAT: 

His DD Form 214, Report of Separation from Active Duty, be 
corrected to reflect award of the Presidential Unit Citation 
(PUC).

APPLICANT CONTENDS THAT:

His DD Form 214, Armed Forces of the United States Report of 
Transfer of Discharge, was never completed at the time of his 
discharge.  

In support of his appeal, the applicant provides a personal note 
and, copies of his DD Form 214 and Special Orders for award of 
four AMs and one DFC.  

The applicant’s complete submission, with attachments, is at 
Exhibit A.

STATEMENT OF FACTS:

The applicant is a former member of the Regular Air Force who 
served on active duty from 11 January 1967 to 21 November 1970.  
His DD Form 214 reflects he served 3 years, 10 months, and 
11 days on active duty with 3 years, 1 month and 11 days of 
Foreign Service.  

After a thorough review of the applicant’s military personnel 
records, the Air Force Personnel Center Directorate of Awards 
and Decorations verified the applicant’s entitlement to the Air 
Medal with three Bronze Oak Leaf Clusters (AM w/3BOLC), Air 
Force Outstanding Unit Award with Valor and two Bronze Oak Leaf 
Clusters (AFOUA w/V & 2 BOLC), Air Force Good Conduct Medal 
(AFGCM), Vietnam Service Medal with two Bronze Service Stars 
(VSM w/2BSS), and the Republic of Vietnam Gallantry Cross with 
Palm (RVNGC w/P).  

DPSID will correct the applicant’s record upon Board action.

AIR FORCE EVALUATION:

AFPC/DPSID recommends denying the applicant’s request for the 
PUC.  DPSID states that after a thorough review of the 
applicant’s master personnel records, they were unable to verify 
the applicant served with a unit that received award of the PUC 
while he was assigned to the unit; therefore, rendering him 
ineligible for award of the PUC.  To grant him the PUC would be 
contrary to the eligibility criteria established by Department 
of Defense Manual 1348.33.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:

A copy of the Air Force evaluation was forwarded to the 
applicant on 5 August 2013, for review and comment within 30 
days (Exhibit D).  As of this date, this office has received no 
response.

THE BOARD CONCLUDES THAT:

1.  The applicant has exhausted all remedies provided by 
existing law or regulations.

2.  The application was not timely filed; however, it is in the 
interest of justice to excuse the failure to timely file.

3.  Insufficient relevant evidence has been presented to 
demonstrate the existence of error or injustice.  We took notice 
of the applicant's complete submission in judging the merits of 
the case; however, we agree with the opinion and recommendation 
of the Air Force office of primary (OPR) responsibility and 
adopt its rationale as the basis for our conclusion that the 
applicant has not been the victim of an error or injustice in 
regard to award of the PUC.  However, we concur with the Air 
Force OPR’s findings to administratively correct the applicant’s 
record to reflect award of the DFC, AM w/V & 3BOLC, AFOUA 
w/2BOLC, AFGCM, VSM w/2BSS, and RVNGC w/P.  Therefore, in the 
absence of evidence to the contrary, we find no basis to 
recommend granting the applicant’s request for award of the PUC.

THE BOARD DETERMINES THAT:

The applicant be notified that the evidence presented did not 
demonstrate the existence of material error or injustice; that 
the application was denied without a personal appearance; and 
that the application will only be reconsidered upon the 
submission of newly discovered relevant evidence not considered 
with this application.
